Tickets are $50 (including GST) per person for the 9th of December

For Care Coordination and e-referral workshop - 9th December
AND
Sharing records and communication - international case studies seminar (10th December):
Tickets are $200 (including GST) per person for BOTH days 9th and 10th December


When registering and paying please put the name and organisation for each registrant in the comments area.

To pay using your credit card:

1. Choose a number of tickets from the form on the right.  Put the name of each registrant with organisation in the comments field.
   
2. Review the subtotal and click "Pay now".  You will be taken to the secure Paypal website.
   
3. You do NOT need a Paypal account to use this facility.
4. If you wish to use an existing Paypal account to make the payment, use the form in the right hand column of the Paypal page
   
5. Fill out the details
   
6. Note: Your address does not have to be a residential address and can be a P.O. Box.
7. Note: Your phone number does not have to be a home phone and may be a mobile. 
   
8. Click "Review Order and Continue".
9. Review your order then click "Pay Now", click "Finalise Payment and Return". You will be brought back to the MSIA website. You wil receive an email tax receipt.
